His goal was that the audience should experience her pain through Madhubala’s face and movements. For this, the water used during the actual shooting is very cold, so much that the body will swell.
But it was not just acting, it was true. Because Madhubala was already a heart attack. The hole in her heart was the ventricular septal defect. The doctor had advised her to rest. The body becomes weak, there was stress on the lungs. But as an artist, she lived the role. She did not even think about stopping.

While standing in that cold water, her feet were swollen, her limbs were shaking, but she never complained on the set. Her health deteriorated after that scene, but she did not have the courage.
